Membership Fees

Enrollment:

​

There is a non-refundable enrollment fee of $100 per household due at your initial appointment.

​

Individual member plans:

​

  • Individual adult aged 21 and older, $100 per month

  • Individual aged 15-20 years, $75 per month

​

Family plans:

​

  • Adult couple, $175 per month

  • Each child aged 15-20, $50 per month up to a maximum of $250 per household per month

​

Active duty military, veterans, and first responders receive a 10% discount (with valid ID or DD-214).​​

HSA/FSA Accounts: Please note, the tax laws are not entirely clear regarding whether direct primary care services qualify for payment with your HSA/FSA card (although several online sources have interpreted the laws to suggest that they do). We are happy to use your card to pay your fees, but we strongly recommend you check with your plan or tax professional to ensure our services "qualify".

We offer a complimentary 15-minute "meet and greet" appointment for you to get to know us, and answer your questions about our services.
